Friends and Companions Adult Day Health Care is Hiring a Case Manager
Friends and Companions Adult Day Health Care (a sister company of Professional Home Health Care Agency) is accepting applications for a Case Manager to enhance the quality of participant management, maximize satisfaction and promote cost-effectiveness. The Case Manager shall be accountable for providing case management to the participants of the Home and Community Based Service program per 907 KAR 7:010 Section 8.
The Case Manager position is responsible for working with participants and their families to develop a Person-Centered Service Plan (PCP) to address identified needs, ensuring services address person-centered outcomes, and are provided in the participant’s environment of their choosing.
QUALIFICATIONS: (Required education and experience are modified by Appendix K per DMS)
- A Licensed Practical Nurse (LPN); OR
- Be an individual with an associate degree: OR
- An individual who has relevant experience to substitute for a degree in provider guidance.
- Maintains current American Red Cross or American Heart Association – CPR and First Aid Certification.
- Must be able to travel and work in varied home environments in all seasons of the year.
- Must have a current driver’s license and the ability and willingness to travel and reliable transportation.
- Position requires the physical ability to perform the job functions outlined in the Case Manager job description.
