We aim to come alongside our clients to help them overcome obstacles, achieve their goals and develop supportive relationships in their family and community. Our services include: Behavior Health consultations for individuals and professionals* Comprehensive Needs Assessments* Ongoing Client Monitoring and Assessments* Developing Individualized Care and Crisis Plans * Referral For and Coordination of Services *Community Based appointments Including House Calls * Evaluation of Progress Toward Individual Goals * Individual Behavior Health Counseling *Intensive Outpatient Programs * Targeted Case management *Dui intervention and education programs
